Handicapped Parking Placard or Plates

Indiana allows those with visual impairments to receive a handicapped parking placard or handicapped plates. Due to the loss of visual acuity, many patients may not be able to walk long distances unassisted or be able to see ice or other hazards when walking through a parking lot. Your doctor can fill this form out and then you can submit it to your local BMV branch for your placard or plate.
